Chase just announced some unique wellness offers for Chase Sapphire Preferred® Card and Chase Sapphire Reserve® cardholders. Through May 31, cardholders can receive a statement credit of up to $69.99 for a Calm Premium membership, $300 back with a Lifeforce diagnostic, or $600 back with a Prenuvo MRI scan. Here are the details.
Chase Wellness Offers
Eligible cardholders can access these offers through Chase Offers via their online account, the Chase app, or clicking on an offer received by email. Ensure you activate the offer before making any payments. Only payments made directly with the merchant are eligible. As usual, any payments must post by the end of this promotion (May 31, 2025) to receive the statement credit.
Up to $69.99 back with the Calm App
Calm is the number one sleep and meditation app, reportedly with at least 40 million downloads and 4 million paid subscribers. The app offers over 500 sleep stories, soundscapes, and guided sleep meditations to help users relax, fall asleep quicker, and generally promote a happier and healthier planet.

Anyone can get a 14-day free trial of the Calm App. Thereafter, users must purchase a Calm Premium subscription for $14.99 per month or $69.99 annually. But for a limited time, Chase Sapphire cardholders can get up to 100% back on Calm subscriptions.
Sapphire Preferred holders can get a $34.99 statement credit after paying the $69.99 for an annual Calm Premium subscription by May 31, 2025. That's 50% back on an annual membership.

Better yet, Sapphire Reserve cardholders will get a $69.99 statement credit with an annual Calm Premium subscription. That's one free year of Calm Premium after the statement credit.
The offer is only valid on the first Calm app payment. As such, the benefit should be available for one annual membership payment for both existing and new Calm users as long as they complete the payment by May 31, 2025.
$300 back with a Lifeforce one-time diagnostic
Lifeforce is a comprehensive, proactive health program that combines diagnostics, doctors, coaches, and science-backed therapies. Members receive an at-home phlebotomist blood draw to test over 50 biomarkers. This is followed up with a telehealth consultation with a clinician and a personalized health program and recommendations. Users have the option of purchasing a one-time diagnostic for $599 or a Lifeforce membership for $349, plus a $149 monthly fee.

An even more unusual offer is available for Chase Sapphire cardholders who want to get a one-time diagnostic with Lifeforce. Both Sapphire Preferred and Sapphire Reserve cardholders will receive a $300 statement credit with the purchase of a Lifeforce one-time diagnostic for $599 by May 31, 2025. This is a 50% savings.

$600 back with a Prenuvo full-body MRI scan
Prenuvo is a state-of-the-art, full-body MRI scan service that offers early detection of potential health conditions. The scan assesses the head, neck, chest, abdomen, pelvis, and legs and checks for solid tumors, metabolic disorders, non-cancerous conditions, spinal degeneration, auto-immune disorders, and brain aneurysms.

The whole-body scan is $2,499 in the U.S. or $2,999 CAD in Canada. You can also choose an enhanced screening which includes the full-body scan from $3,999, a head and torso scan for $1,799, or just a torso scan for $999.
Sapphire Reserve cardholders who spend $2,499 or more for the full-body scan by May 31, 2025, will get a $600 statement credit. That's more than the card's annual fee! As the offer is “$2,499 or more”, you could also get the statement credit with the enhanced screening.
This offer is only available to Sapphire Reserve holders. Sapphire Preferred cardholders aren't eligible.

Our Take
As someone who takes my health seriously, I'm quite excited about these new Chase wellness offers. I've always been interested in Calm, and I'd go for the full-body MRI in a heartbeat. Sadly, I don't hold the Sapphire Reserve.
Minimally, I would recommend that all Sapphire Reserve cardholders get an annual Calm membership. After all, cardholders get a free yearly membership after the statement credit. If you are interested, have the wherewithal, and have the right card, I would highly recommend taking advantage of the other two wellness offers with Lifeforce and Prenuvo.
